Supercharge value-based care performance

Catholic Medical Partners (CMP) has been a partner of Vatica Health since the beginning of 2015. CMP’s 230+ primary care providers treat 19,000+ patients across multiple health plans. With such a large patient population, it was important to integrate Vatica’s solution to maintain a high standard of coding accuracy and documentation and overall patient-provider relationship.

Goals

CMP needed a solution that complimented its EMR, as well as one that provided a comprehensive view of patients while facilitating thorough visits and enhanced coding and documentation. Our technology helped CMP achieve these goals.

  • Succeed in value-based care (VBC) arrangements
  • Improve quality of documentation to drive coding accuracy and specificity
  • Close open quality gaps
  • Improve the rate of preventive/wellness visits among the eligible population

Solution

Providers at CMP use Vatica Health to improve documentation and coding for risk adjustment, close gaps in care and increase the utilization of preventive services.

  • Provides dedicated nurses, or Vatica Clinical Consultants (VCCs), to conduct a thorough search of the EMR for relevant clinical information and synthesize it with health plan data
  • Supercharges the EMR to surface appropriate, clinically validated codes
  • Positively impact the closure of open care gaps (e.g. breast cancer, screening, colorectal cancer screening, and anti-rheumatic drug therapy, etc.)

Results

Patient engagement

77%

of eligible members are better coded and better risk-adjusted.

Revenue

AWV + Incentive x Volume

$150 + $150 x 72,000

Completed more than 72,000 Vatica supported visits, earning a substantial increase in additional revenue.

Chronic condition coding

Over 100,000

clinically validated chronic conditions submitted through the Vatica program, driving more complete documentation and accurate scores.

“My favorite part about working with Vatica is the team is always with me. Lots of times you hire people to help you, they’re there in the beginning, and then they become vaporware. Vatica is always so responsive. It’s been incredibly important to us.”

Margaret Paroski, MD, Catholic Medical Partners
