New CMS HCC model (V28)

The V28 model reorganized the entire ICD-10-CM code set which now maps close to 74,000 ICD-10 codes to 266 HCCs. V28 has approximately 2,000 fewer ICD codes eligible for risk adjustment. With fewer codes, it’s more important than ever to code accurately and completely for MA patients. And with CMS’ increased scrutiny, these codes need to be documented and substantiated.

V28 has some notable new conditions that are eligible for risk adjustment. Remember to document and code these conditions annually.
